formats: PDF, JPG or PNG). Make sure the exemption certificate is signed and

dated. Please note, copies of registrations, licenses, permits, W-9's and IRS

letters are not considered exemption certificates and will not be approved.

My company is tax-exempt. What do I need to provide so I am not charged

tax?

If your organization is exempt from sales tax, please provide a properly

completed sales tax exemption form from your state. The form should be

completed in its entirety and be signed and dated. You can upload the form

here.

Do I need to submit a tax-exempt form each year?

Not necessarily. Exceptions to this would be changes to your business that

necessitated a new exemption form to be issued.
